Application Window for Warm Neighbors Fund Will Open Dec 15, 2025

Applications will be available to download below once window opens.

If you are not able to apply online, paper applications will also be available from the folowing community organizations who have partnered with CAHP to ensure the campaign reaches our most vulnerable residents: the Phoenicia Food Pantry, the Phoenicia Farmer’s Market, the Phoenicia Library, the Pine HIll library, Catskill Neighbors, the Pine Hill Community Center and Wellness RX.

Grants will be awarded beginning January 5, 2026, with priority given to applicants who can prove they have been eligible for SNAP or HEAP benefits at any time in the past 2 years. The amount of each grant will be determined based on the overall amount of money raised, with a $500 maximum grant per household.
